11. Chrome Extension Data Use Disclosure

This section applies to the Phil-Am Client Intake Chrome Extension ("the Extension"), used exclusively by authorized Phil-Am Insurance Agency team members. Last Updated: June 25, 2026.

Purpose of the Extension

The Extension helps Phil-Am Insurance staff record client intake directly within the GoHighLevel (GHL) platform across our lines of business — Medicare, ACA, Life, Annuities, and Ancillary. From a contact record, authorized staff can view and update the client's information and the related sales opportunity.

Data the Extension Handles

When a staff member opens the Extension on a contact, it reads and writes that contact's data so it can be reviewed and updated. This may include personally identifiable information (such as name, address, email, phone, and date of birth) and health-related information relevant to insurance enrollment (such as Medicare or Medicaid identifiers and plan details). The Extension only handles data for the specific contact a staff member chooses to open, and notes entered by staff are posted to that contact's timeline in the CRM.

How Data Is Transmitted

The Extension communicates only with a backend service operated by Phil-Am Insurance Agency, which relays requests to GoHighLevel. This backend transmits data to and from GHL solely to perform the Extension's function; it does not maintain a separate store of client records, and no client data is shared with any other third party. Data is never used for advertising, behavioral profiling, credit or lending determinations, or any purpose unrelated to the Extension's function.

No Tracking or Analytics

The Extension does not track browsing history, user actions, mouse movement, keystrokes, usage analytics, system information, or website content. No data is shared with advertisers or used for behavioral profiling.

Permissions Used

The Extension uses only the minimum permissions necessary to function: a content script on our GoHighLevel domain to add the intake button, and a single host permission limited to Phil-Am's own backend service.

Internal Use Only

The Extension is distributed as an unlisted item and is intended exclusively for authorized Phil-Am Insurance Agency team members. It provides no functionality outside the GoHighLevel CRM environment.
